There are two options. You can download the paper version from the printed brochure in PDF format, or you can register online.- Print and fill out the Brochure Version as published.
Even though you may get a bad font message for the check boxes, they seem to print fine.
If you have a MAC, you could also open in Preview; that should eliminate the problem.
Be sure to sign the form for credit card payment.
Mail with your check, credit card information, or purchase order to the address on the form.
