Lurie Center Application

  • Application shall include a letter from the applicant stating the projected benefits of attending the conference and how the applicant encourages students to pursue mathematical literacy.

  • Application shall include a letter of recommendation from a supervisor or colleague, addressing your encouragement of students to pursue mathematical literacy.

  • Applicant's Signature on this statement:
I certify that I meet the scholarship requirements.
  • Supervisor's Signature on this statement:
    I certify that the above teaching assignment is correct and that I will provide this teacher the necessary release time for conference travel and attendance.
  • Supervisor's name and position

Please note the due date for applications is approximately 5 weeks before each section conference as listed.
        Oct 1 for South, Palm Springs
        Nov 1 for North, Asilomar
        Feb 1 for Central, Seaside

Be sure to include a facsimile of this form, your letter, and the recommendation letter.

Incomplete applications will not be considered.

You will receive a questionnaire regarding the educational materials purchased and an evaluation of the benefits of conference attendance and CMC membership. You must complete this to receive reimbursement for conference expenses.
