2010 Registration Information
Type of Registration
- Member:
Have your member number to enter
onto the registration form. It can be found in the corner of the address label
of the Communicator. If you do not
know your member number, you can still register; we will look it up for you
after you register.

Non-Member:
You will receive a one-year membership in CMC with your registration.

Student: To
register as a student, you must be a full-time student and not receiving pay as
a teacher.

Speaker/Co-Speaker:
Speakers receive complimentary registration that includes a one-year
membership. Co-speakers (person listed second on the program) receive
complimentary registration but must pay $50 for membership
Group Registration

You
will have to identify a contact person for the group. This person does not have
to attend the conference. The contact person should be listed in the billing
information section.

Up
to five attendees can register on one form. After you have entered the personal
information and added it to the cart, you can delete the information and enter
the next attendee’s information.
Payment Options

Credit
Card: Have your credit card
when you begin registering. Credit card payments are made through PayPal even
without a PayPal account. If you select credit card, you will be taken to the
PayPal site as soon as you finish your registration form. If you have
difficulty with the PayPal site, do not re-register. Contact us at palmsprings@cmc-math.org. You will be
sent an invoice that will allow you to make your payment.

Personal
Check: After completing your
registration, mail personal checks to Cheryl Avalos, 3109 Sunrise Ridge Lane,
Hacienda Heights, CA 91745. Checks should be made out to CMC-South.

Purchase
Order: Purchase orders must be mailed to Cheryl Avalos, 3109 Sunrise Ridge
Lane, Hacienda Heights and an invoice will be sent to your district. You must
enter a contact person and contact information. Enter the PO number if you know it. You may register even
though you do not have the PO number or a copy of the purchase order. However,
you will not receive your registration materials until a copy of the PO is
received.
Ticketed Sessions
Sessions
that are scheduled in smaller rooms (less than 50) are ticketed. Indicate any
ticketed sessions that you want. You can select two ticketed sessions and two
alternates. You will receive your tickets with your registration materials. Due
to the limited number of tickets, many people do not receive tickets. There are
many wonderful non-ticketed sessions in large rooms available during each
session time.

Invoice: You
will receive an invoice when you complete your registration. You may keep the
invoice as a receipt or turn it in to your district to be processed with your
purchase order.

Cancellations:
If you must cancel, please notify CMC-S in writing. Your money will be refunded except for $50 membership fee and a $20
processing fee. If you have received the registration materials, they must
be returned with your notice. Cancellations will not be honored after November
1, 2010.

Conference Hours: 7:30 a.m. Friday to 5 p.m. Saturday
On-Site Registration: 7:30 - 4:30 Friday; 7:30 - Noon, Saturday
Exhibits: 8:00 - 5:30 Friday; Saturday 8:00 - 3:00
